I'm so glad you're here!

End-of-life psychotherapy is sacred work. It invites us into sensitive, complex terrain—grief and love, autonomy and protection—and sometimes into conversations about Death with Dignity/Medical Aid in Dying. Systems don’t always make space for this depth, but you can.

As therapists, social workers, psychologists, and aging-services professionals, you are a secure base for clients, families, and teams. You are uniquely positioned to hold complexity and fear, and help people navigate ethical dilemmas with clarity and compassion.

I’m Dr. Regina Koepp, a Stanford-trained, board-certified clinical psychologist and geropsychologist, and founder of the Center for Mental Health & Aging. For more than two decades across major health systems, ethics committees, and collaborative care teams, I’ve helped professionals deliver trauma-informed, ethically sound care to medically vulnerable adults.

This training offers practical tools and a steady framework to honor self-determination, dignity, and safety—while supporting people who are dying and their families with tenderness. I’m grateful you’re here. Together, we can ensure older adults receive expert, compassionate care at the end of life.

About the Course

This 6-hour intermediate ethics course will challenge conventional thinking about the rights of older adults living with terminal illness and separately older adults with dementia disorders. Participants will learn three (3) models of psychotherapy for use at the end of life and gain tools for differentiating desire to hasten death when living with a terminal illness from suicidality. In addition, the right to die, Medical Aid in Dying (MAID)/Death With Dignity (DWD) movements will be explored, including ethical considerations and dilemmas, and therapists will discover unique ways that they may support people with MAID. This webinar will also explore the topic of aggressive advance directives and discuss ethical considerations and philosophical problems related to dementia end of life care, including the questions: Is someone with severe dementia the same person as they were prior to this state? Are people living with dementia allowed to evolve, change, and make decisions for their own life?

After attending this intermediate-level program in full, participants will be able to:

  • Describe three (3) therapy modalities for working with patients at end of life.
  • Summarize the Right to Die movement and Medical Aid in Dying/Death With Dignity
  • Identify a model for differentiating suicidality versus desire to hasten death among patients living with terminal illness
  • List 4 ways therapists can provide support to people with terminal illness considering Death with Dignity
  • Illustrate 3 philosophical problems associated with advance directives related to dementia end of life care
    • 1. Deepen therapists’ clinical and ethical competence in providing psychotherapy to older adults with terminal illness and dementia, applying three (3) psychotherapy models tailored to end-of-life care.
    • 2. Strengthen clinicians’ ability to differentiate between suicidality and a desire to hasten death among clients with terminal illness, and to respond therapeutically and ethically in ways that promote dignity and alleviate suffering.
    • 3. Increase understanding of ethical, legal, and philosophical considerations in the Right to Die, Medical Aid in Dying (MAID), and Death With Dignity (DWD) movements, including implications for clinical practice and the therapeutic relationship.
    • 4. Enhance clinical skill in supporting individuals and families navigating end-of-life decisions by integrating evidence-informed psychotherapy approaches with ethical decision-making frameworks.
    • 5. Promote reflective practice and self-awareness in working with death, dying, and autonomy, helping therapists examine personal values, countertransference, and societal attitudes toward aging and end-of-life care.

    Course Schedule:

  • 9-10:30 EST (90min): Models of psychotherapy at end of life- living with terminal illness.
