When Memory Loss Appears in Therapy, It Can Get Complicated Fast.

If you’re working with an older client who’s beginning to show memory changes, there’s a moment every therapist faces — UNCERTAINTY

You may find yourself wondering:

✔️ Are my client’s memory changes typical for their age, or signs of a cognitive disorder?
✔️ Should I invite their family or caregiver into therapy — and if so, how can I do that while protecting my client’s dignity and autonomy?
✔️ How do I navigate growing caregiver concerns, family tensions, or disagreements about what’s best?
✔️ How can I adapt therapy so my client feels respected, understood, and safe — even as their abilities change?

These are complex, emotionally charged questions.

And yet, most therapists received little to no training on how to navigate them.

Meanwhile, the choices we make in these moments matter deeply.

They can shape the course of a client’s diagnosis, influence family relationships, and affect the client’s quality of life and independence.

That's why I created Therapy with Dementia Disorders — a practical, heart-centered, 6-hour continuing education workshop that bridges the gap between what we know about dementia and cognitive decline and what we actually do in the therapy room.

You'll learn evidence-based therapy for people living with dementia and their families, and build clinical skills for every stage — from early memory loss and mild cognitive impairment through advanced stages of dementia to end of life.

Because your clients deserve skilled, compassionate care at every step of the journey. And you deserve the training to provide it.

What You'll Learn in This Workshop

Therapists are increasingly on the front lines of dementia care — yet most received little to no training on how to respond. This workshop closes that gap. You'll leave clinically confident, practically equipped, and ready to provide skilled, compassionate care at every stage of the dementia journey.

In this course, you'll learn:

✔️ Typical vs. Atypical Brain Changes With Aging

How to tell the difference between normal aging and red flags — and how to talk with clients about cognitive change without alarming them.

✔️ MCI, Dementia, and Common Dementia Disorders

What to expect clinically, functionally, and relationally — from mild cognitive impairment through advanced dementia.

✔️ The Mental Health–Dementia Connection

How depression, anxiety, trauma, and sleep problems increase dementia risk — and why mental health treatment still matters across the full trajectory.

✔️ Cultural Considerations and Health Disparities

Equity-informed practice: race, ethnicity, LGBTQIA+ considerations, medical mistrust, and culturally responsive assessment and referral.

✔️ Neuropsychiatric Symptoms and Differential Diagnosis

Differentiating dementia, depression, and delirium — and recognizing apathy, agitation, anxiety, and other symptoms that surface in the therapy room.

✔️ Assessment Toolkit for Therapists

Cognitive and mood screeners, functional assessment, ethical collateral input, documentation essentials, and when to refer.

✔️Psychotherapy With the Person Living With Dementia

Therapy adaptations for every stage — including life review, reminiscence therapy, identity and dignity work, and responding to suicide risk after diagnosis.

✔️ Caregiver Family Therapy

Work with reluctant, resentful, and burned-out caregivers. Help families understand behavior, set boundaries, and build sustainable support plans.

✔️ Caregiving Dilemmas and Compassionate Decisions

Navigate the hardest conversations — medication management, driving cessation, independent living — while honoring dignity and balancing safety.

About the Course

Therapists are often the first to notice cognitive changes—and to hold the fear, grief, and family tension that follow. This workshop helps you distinguish typical brain aging from warning signs of mild cognitive impairment and dementia, and understand how depression, anxiety, trauma, and sleep problems can mimic or worsen cognitive symptoms—and why treatment still matters.

You'll leave with a practical, therapy-ready assessment toolkit: brief cognitive and mood screeners, functional assessment, and caregiver or collateral input, plus a clear approach to differentiating dementia, depression, and delirium. We will cover evidence-informed psychotherapy adaptations across stages, including life review and reminiscence therapy and caregiver and family therapy, and address common clinical themes: ambiguous loss, neuropsychiatric symptoms (apathy, agitation, anxiety), increased suicide risk after diagnosis, and helping the person living with dementia maintain meaning, identity, and dignity.

You will also learn strategies for working with reluctant, resentful, or burned-out caregivers, and for helping families have difficult conversations—assessing when collaborative decision-making is possible, and when to pivot to supporting caregivers in making compassionate safety decisions about medication management, financial management, driving cessation, and independent living, while doing the best to honor dignity. Finally, you will receive a clear referral roadmap and key community resources.

  • Life review and reminiscence therapy, validation-based responses, strengths-based meaning work
  • Supporting identity, dignity, and a meaningful life despite cognitive losses

  • Stages of the caregiving journey and common points of conflict
  • Working with reluctant, resentful, or burned-out caregivers: guilt, anger, boundaries, and support plans

  • Clinical responses caregivers can use: de-escalation strategies, routine and cueing plans, caregiver coaching, distress tolerance tools
  • Referral roadmap: dementia evaluation pathways and key community-based supports and resources
  • Limitations and risks: caregiver overload and burnout; safety concerns when symptoms escalate; need for medical evaluation for sudden changes

9) Caregiving Dilemmas, Difficult Conversations and Compassionate Decisions (60min)

  • Helping families and medical teams navigate common dilemmas: medication management, financial management, driving cessation, and decisions about independent living
  • Limitations and risks: conflicting family reports; over- or under-estimating capacity; balancing autonomy, dignity, and safety when stakes are high

By the end of this workshop, participants will be able to:

  • Define 10 typical versus atypical brain and cognitive changes associated with aging, including changes that may indicate mild cognitive impairment or dementia.
  • Differentiate clinical presentations of dementia, depression, and delirium, including how common mental health conditions can mimic or worsen cognitive symptoms.
  • Apply a therapy-relevant assessment toolkit for cognitive concerns, including brief cognitive screeners, mood screeners, functional assessment, and caregiver or collateral assessment.
  • Adapt psychotherapy strategies across stages of dementia, including life review and reminiscence therapy, to support meaning, identity, and dignity for the person living with dementia.
  • Implement caregiver and family interventions to address ambiguous loss and conflict, work effectively with reluctant or resentful caregivers, and guide difficult conversations toward compassionate safety decisions (medication management, financial management, driving cessation, and independent living).
  • Increase therapists' confidence and competence in recognizing cognitive red flags and responding clinically and ethically.
  • Strengthen differential diagnosis skills so therapists can respond appropriately to cognitive symptoms in the context of mental health treatment.
  • Improve clinicians' ability to integrate efficient, evidence-informed assessment practices into routine psychotherapy workflows.
  • Expand therapists' intervention repertoire for supporting both the person living with dementia and family caregivers across the stages of dementia.

    I'm so glad you're here!

Hi there, I’m Dr. Regina Koepp. I’m a Stanford trained, board-certified clinical psychologist, geropsychologist, and founder of the Center for Mental Health & Aging. I’ve spent over 20 providing therapy to older adults and their families addressing issues like memory loss, dementia, and caregiving.

I created this course for a simple reason:

Because I’ve seen too many therapists—brilliant, caring therapists—feel helpless when memory loss enters the room.

    They weren’t trained in this.

    They worry they’re missing something.

They don’t know what to say—or where to turn.

And I get it. I’ve been there too.

This course is everything I wish I had when I started working with older adults more that 20 years ago. It’s practical. Compassionate. Clear.

And it gives you the tools to support older clients with memory loss—with confidence and heart.

Because your clients deserve the very best care, and YOU deserve the very best training.
